Going to a restaurant is a nice idea, but it depends how much effort it is for her - would it be better to set a nice table at her house and do it there? That would assume that she has a table big enough, perhaps was "proper" enough to have a nice tablecloth and tableware. She might enjoy using some of her nice things if she has kept them. I would think about brunch or lunch instead of dinner, and look for something you could make ahead and heat up. Or - would she enjoy baking something with you? Her favorite cookie recipe that she could show you how to make (and you would do the heavy lifting). Ask her if there's anything that she'd like you to help her with at her home.
