I think this book could be My Mama Says There Aren't Any Zombies, Ghosts, Vampires, Creatures, Demons, Monsters, Fiends, Goblins, or Things. It's got very creepy black and white illustrations. The boy in the book is imagining all sorts of different monsters in his mother's house. No reference to hell or the River Styx that I remember.
