Your wife is pretty sweet to be ok with you having that squeezed in next to the tv like that. That’s the kinda two way street I like to see. I think a monitor arm would definitely open things up, as well as a smaller case. I imagine there’s no way to keep the pc off the desk altogether. I’ll say this, though. If you do get a smaller case, going for white is going to create a less cluttered illusion. You could get a nice looking case that would appear decorative on the entertainment console below the tv (maybe a black case in that situation, so it blends in a bit?), then use a cable raceway of sorts to keep those nice and hidden, run it along the back of the desk top, to behind the drawers where the power strip will be. That hides those wires away instead of keeping them on your desk, and opens up more space with the pc itself being on another surface. That picture above the setup actually clutters the space more because it’s too small to be alone, and too low, while also not being centered. Your eye is drawn there and it doesn’t look cohesive.
