We use washable incontinence pads under cheap brown blankets (I did this for years just to keep our dogs' doggy-stink out of the sofa), so you might add those to your underlayer at least. They've suddenly started making super cute pads now if you want to put them on top of the quilts. How often are they getting new stains? That's really where my concern is, I don't care about old washed-in discolorations as much as bodily fluids just sitting there drying on it.
