It will vary by person and dosages. If someone is not deficient in B12 then they may not need to supplement B12 or just take 2-10mcg of B12. For a small subset of people, they may find R5P too stimulating and need to use plain riboflavin; others start with just a dab of R5P powder from a capsule and increment up slowly over time. P5P often does not need to be supplemented. Using a children's multivitamin is another option that allows one to slowly increment up multiple nutrients starting from a low level by using 1/2 or even 1/4 dose to start.

Avoid methylated vitamins for now if possible. I'd suggest a good children's multivitamin like Seeking Health Kids Multivitamin MF. One dose is two chewable tablets, so you could start with one table or just a half a tablet, and then slowly increment up the dose over time. I did not see this product on Amazon.de so you may have to find an alternative. DAO is the enzyme that breaks down histamine in the gut. It requires adequate copper and calcium to produce it. There are DAO supplements available, which can be helpful. Liver can be a good source of nutrients including choline, vitamin A, and copper, but you want to start with small doses (e.g., size of a pea) and see how it goes. If ok, then increment up the amount slowly over time. Egg yolks are also good sources of choline and other nutrients. Sometimes the egg whites can cause histamine issues. The panic/derealization may be from a buildup of acetaldehydes at the third step in histamine breakdown; this step requires B3, so gently increasing niacin may help with that.

For MTHFR you have heterozygous C677T, your MTHFD1 is ok, and your PEMT is homozygous. With that combination, you would need about 900mg of choline to compensate for the variants. This can take the form of getting 500-550mg of choline from your diet plus a 750mg capsule of TMG. This allows the choline/TMG dependent pathway to compensate for the reduced bandwidth in the methylfolate/B12 dependent pathway. You may need to start the TMG by opening the capsule and taking just a tiny fraction at first, and slowly increment up over time, in order to avoid those anxiety side effects that you get. However, Genetic Lifehacks does not list the values of your SLC19A1 variant, which can further decrease the pathway bandwidth. You may want to consider starting with Seeking Health Children's MF multivitamin. The "MF" means "methyl-free", and being for children, it is low dose. Each dose is two chewable tablets, so you can start with one tablet or break a tablet apart for an even smaller dose. Eventually you should be able to tolerate the adult Multivitamin One MF.
