I do. You have to go into settings and create a couple of templates/instructions to make it effective. A couple of mine are “always document the names of any physicians mentioned by the patient” in the HPI, otherwise it’ll just say “they reported following up with their PCP LAST Thursday” instead of “their PCP Dr Sanjay Singh”. I also have a generic “no touch” exam that I tell it to insert at the end. When I’m walking to my desk after the encounter I just say “Cleo, add my no touch physical exam in addition to <findings>” although I verbalize most of my exam while it’s happening. A lot of time it over simplified the MSK exam, so I have instructions to transcribe it verbatim, such as “positive pain with resisted internal rotation of arm, but not with resisted external rotation. No pain with passive internal rotation. Positive pain with passive and resisted lateral abduction, positive speed test,” etc. otherwise it sometimes simplified it to “pain with movement of the right shoulder joint. It also has an overcomplicated EKG documentation prompt I that I tell it to ignore and just transcribe my EKG verbatim as I dictate it. I also recommend adding your most common scoring systems in a nice format, such as the HEART score, PECARN/CHALICE head injury rules, NEXUS cervical spine imaging rule, and an instruction to list all the criteria and which one is positive to justify why you ordered imaging. If you have a SEPSIS nazi at your facility, putting a sepsis template in your MDM section can be helpful to make sure everything is documented, and ordered correctly. Same with Workman’s comp if your location has strict follow up criteria. It records the sound so much better than your phone in your pocket. I dictate into the mic my plan as I walk to my desk. Just make sure to close out your encounter soon after. I’ve had several times where it adds lines because a coworker is dictating near me and it added their H&p or findings to my notes and that’s annoying to fix.
