Before my hands got worse, some combination of compression gloves and a not completely rigid thumb brace (not really sure what to call it but the one I bought is here) tended to help. Taking frequent breaks, crocheting with a pillow in my lap, loose tension, and wrapping the yarn around my hand and wrist for most of the tension rather than relying on my fingers was also super helpful.
